GROUP FITNESS CLASSES ONLINE

If at any time during the class Live Virtual classes, you feel discomfort or strain, stop the activity or gently come out of the posture. You may rest at anytime during the class on or offline. It is important in your fitness and yoga classes that you listen to your body, and respect its limits on any given day.

I understand that the fitness and yoga programs includes physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension.

As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask or reach out for support from the instructor.

I will continue to breathe smoothly.

I understand that Fitness and Yoga are not a substitute for medical attention, examination, diagnosis or treatment.

The Fitness and Yoga programs are not recommended and are not safe under certain medical conditions. By signing up to this membership, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness programs, if required.

If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician’s approval to participate.

I also affirm that I alone am responsible to decide whether to practice these fitness / yoga programs and participation is at my own risk.

Intrinsic Mind may share snippets of video footage and photos for Marketing purposes.

If any portion of this release from liability shall be deemed by a court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.

Confidentiality Agreement & Liability Waiver – Human Design Reading
Agreement between yourself and Leisa Timss of Intrinsic Mind specialist agrees to provide a Human Design Reading Service for the Client.

Description: A Human Design Reading is a partnership (defined as an alliance, not a legal business partnership) between the Specialist and the Client in a thought-provoking, educational, informative process that inspires the Client to maximize and expand personal and professional potential.

Confidentiality: This Specialist to Client relationship, as well as all information (documented or verbal) that the Client shares with the Specialist as part of this relationship, is bound by confidentiality, but is not considered a legally confidential relationship (like in Medicine or Law). The Specialist agrees to not to disclose any information pertaining to the Client without the Client’s written consent. The Specialist will not disclose the Client’s name as a reference without the Client’s consent. Confidential information does not include information that: (a) was in the Specialist possession prior to its being furnished by the Client; (b) is generally known to the public or in the Client’s industry; (c) is obtained by the Specialist from a third party, without breach of any obligation to the Client; (d) is independently developed by the Specialist without use of or reference to the Client’s confidential information; or (e) that the Specialist is required by law to disclose.

Liability: Client is responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results. As such, the Client agrees that the Specialist is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Specialist.

Client understands that a reading is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease. Client understands that a reading is not to be used as a substitute for professional advice by legal, mental, medical or other qualified health professionals and will seek independent professional guidance for such matters. If Client is currently under care of a mental health professional, Specialist will recommend that Client inform the mental health
care provider.

Client agrees to communicate honestly and be open to feedback and assistance. This is the entire agreement of the parties, and reflects a complete understanding of the parties with respect to the subject matter. This agreement supersedes all prior written and oral representations.
